Contact Your Insurance Company

It is recommended that you contact your insurance provider prior to scheduling any services to ensure that you are aware of your coverage and anticipated cost - deductibles, copays, co-insurance, etc. Below is a helpful script to use when contacting your insurance provider: 

“Hello, I’m calling to clarify coverage for mental health services and confirm that (provider name) at Ellison Center is within network. Can you please let me know if I have any deductible, copay, or co-insurance costs? If that provider is working as an intern or is under supervision are they also covered as within network?”

Sliding Fee Scale

It is the policy of Ellison Center to provide essential services regardless of the patient's ability to pay. Discounts are offered based on family size and annual income.  Please reach out to our admin team for a sliding fee scale application to determine if you or your family members are eligible for a discount. The discount will apply to services received at this clinic. The application must be completed every 12 months or if your financial situation changes.

NO ONE WILL BE DENIED ACCESS TO SERVICES DUE TO AN INABILITY TO PAY.

A SLIDING FEE SCHEDULE OF DISCOUNTS BASED ON FAMILY SIZE AND INCOME IS AVAILABLE.
